Intention of Family Practice physicians to leave Primary Care in Florianópolis and associated factors

Abstract

Introduction: A major challenge for the implementation of a quality primary care in Brazil is the turnover of professionals, especially physicians, which significantly harms the attribute of longitudinality. Florianópolis has already been recognized for its high coverage of the Family Health Strategy (FHS) and a high proportion of family practice physicians (FPP) in its network. However, the recent increase in the number of physician resignations poses a threat to the continuity of care in this municipality. Objective: To assess the intention FPP to leave primary care Florianópolis within the next 5 years and to identify possible associated factors. Methods: A cross-sectional study was conducted between November and December 2023, using virtual questionnaires administered via the KoboToolBox platform. Job satisfaction and its domains were measured using the adapted Warr-Cook-Wall scale. Subsequently, the association of variables with the outcome “intention to leave” was tested through two-tailed statistical tests, with an alpha level of 0.05. Results: A total of 95 responses were obtained, equivalent to a response rate of 69.9%. About 42% of FPP intended to leave Florianópolis primary care within the next five years. Variables significantly associated with the outcome included “active population in medical consultations” (p=0.046), “working hours greater than 30 hours” (p=0.001), lower “overall job satisfaction” (p<0.001). Moreover, the domains “physical conditions” (p=0.015), “recognition for work” (p=0.002), “amount of responsibility” (p=0.024), “salary” (p=0.042), “opportunity to use skills” (p=0.002), “amount of variety” (p=0.032), and “working hours” (p<0.001) were also significantly associated with the outcome. Conclusions: There is a high rate of turnover intention among Florianópolis FPP within the next 5 years, which is associated with some of the researched variables. Considering that the intention to leave the job often precedes actual resignation, it is essential to cautiously evaluate the reasons that may lead to the non-retention of FPP to avoid compromising the continuity of care.
